1a. Tell Apple about your organization
Begin the application process by signing in to Apple Business Register, then provide information about your organization.
Note: If your organization has multiple FHIR API endpoints, you should coordinate a single registration for your entire organization. To reduce redundancy and help your teams coordinate their efforts, Apple notifies the administrative and technical contacts if it detects the creation of duplicate registrations.
Sign in to Apple Business Register
Go to the Apple Business Register website.
Sign in to the Apple Account you created for Apple Business Register.
Read and accept the Terms of Use for Apple Business Register.
Click Get Started.
If the list of services for Apple Business Register appears, click the Add button for Health Records, then click Done.
Click Health Records, then click Get Started.
Read and acknowledge the Health Records Acknowledgment statement, then click Agree.
Note: Later in the registration process, you or someone with signing authority will review and accept the separate Health Records Directory Listing Agreement.
Register as a healthcare organization
In the first field, choose Healthcare Organization as the type of organization.
Note: To participate in Health Records, your organization must be a hospital, clinic, physician practice, telehealth provider, medical laboratory, pharmacy, or other organization that provides direct patient care in Canada, the United Kingdom, or the United States.
Use the information provided by your EHR vendor to supply answers in the next two fields: How Many EHR Vendors Do You Have? and How Many FHIR API Endpoints Do You Support?

Click Next.
On the Overview screen, familiarize yourself with the registration process, then click Next.
Register your organization name and address
On the Organization Details screen, click the fields to provide the requested information. As you enter information, a Health app Listing Preview shows how it will appear on iPhone and iPad. You’ll have opportunities to edit this information later.
Organization or Brand Name: Enter the legal name of your organization, such as the name of its corporation or LLC.
Alternative Organization or Brand Name (Optional): If your patients are more likely to search for and recognize your organization by a name other than its legal name, enter another name, such as a brand name or a practitioner’s name. If your patients also know your organization by other brand names, you can register those names later. (See Determine how to brand your organization in the Health app.)
Upload organization logo: Provide a logo to help your patients identify your organization. If you don’t upload a logo, a circle containing the first letter of your organization or brand name is automatically supplied. (See Prepare your logo for the Health app.)
Address: Enter a street address for your organization, such as a location where your patients receive care. If your organization has multiple locations, enter the address for any one of them. Or enter the address of your corporate headquarters. Later, you can add addresses for more locations.
Do one of the following:
If you previously answered that your organization has one FHIR API endpoint: Continue entering the requested information by following the instructions in 1b. List contacts, locations, and brands.
If you previously answered that your organization has more than one FHIR API endpoint: There is no more information to enter on this screen. Click Next, then respond to Health Records Directory Listing Agreement, as described below.
Respond to the Health Records Directory Listing Agreement
A signing authority from your organization is required to review and accept the Health Records Directory Listing Agreement. (See Identify who will fill registration roles.)
To learn more about the agreement while you review it, click Requirements in the Health Records Directory Listing Agreement. To print the agreement, click Print.
With the agreement onscreen, do one of the following:
Read and accept the terms of the agreement: Select the option that begins with “I accept.”
Note: This action can’t be undone.
Forward the agreement to your organization’s signing authority: Select the option that begins “I am not the signing authority,” then enter the requested information. Advise your signing authority to expect an email from Apple with the subject “Health Records Directory Listing Agreement.”
Continue without accepting the terms of the agreement: Select the option that begins “Skip for now.” You can continue configuring FHIR API endpoint information, but as you’ll see, you won’t be able to publish any endpoints until the agreement is accepted.
After you choose one of the three options below the agreement, click Submit, then follow the instructions in 1b. List contacts, locations, and brands.
