Change Template settings in Contacts on Mac
Use Template settings in Contacts to choose the fields that appear on new contact cards. Existing cards aren’t changed when you edit the template.
To change these settings in the Contacts app on your Mac, choose Contacts > Settings, then click Template.
Option | Description |

Add Field | Click the pop-up menu, then choose a field to add to the template. A field with a checkmark is already in the template; to remove it, choose it again. (You can’t remove fields that are dimmed.) |
Add | Click the Add button in the template to allow another entry for a field. The Add button appears only for fields that can have multiple entries, such as Phone. |
Remove | Click the Remove button in the template to remove a field. |
Labels | Click a label, such as home page, then choose a different label for the field. To create a custom label, click Custom, if available. |
To change the order of the first and last name in the template, use the Show First Name option in General settings. See Change General settings.
