Change Template settings in Contacts on Mac
Use Template settings in Contacts to choose the fields that appear on new contact cards. Existing cards aren’t changed when you edit the template.
To change these settings in the Contacts app on your Mac, choose Contacts > Settings, then click Template.

Add Field | Click the pop-up menu, then choose a field to add to the template. A field with a checkmark is already in the template. (You can’t remove fields that are dimmed.) |

Labels | Click a label, such as “home page”, then choose a different label for the field. If a Custom option is available, click it to create your own label. |
To change the order of the First and Last name fields in the template, use the Show First Name option in Contacts General settings. This option also changes the order in all existing cards. See Change General settings.
