Users unable to SSH into OS X Server

Lion Server, Mac OS X Server 10.6, OS X Server (Mountain Lion)

Symptoms

Users may be unable to access the server via SSH. After entering a password three times, a "permission denied" message may appear.

Resolution

Assign the user a Home folder location on the server.
