iTunes syncs with your iPod automatically

iTunes syncs the saved On-The-Go playlists from your iPod as new iTunes playlists. After you sync, you'll see them in the iTunes source list with the names "On-The-Go 1", "On-The-Go 2", and so forth. You can rename them if you'd like. iTunes syncs the renamed playlists to your iPod and removes the duplicate playlists named "New Playlist 1", "New Playlist 2" so you're ready to create and save new On-The-Go playlists.

You choose to manually sync your iPod

iTunes does not save On-The-Go playlists from your iPod as new iTunes playlists. You can't see the On-The-Go playlists on your iPod in the source list and iTunes doesn't sync them for you. If you want, you can create a new playlist with the same songs in iTunes, then sync it to your iPod.
